QUALITY LIVING HOMES LLC will offer services to:

● Males and females 18 years of age and above, who have been diagnosed with a primary intellectual disability and there can be a secondary diagnosis of Mental Illness to include;

● Bi-polar disorder,

● Schizophrenia,

● Anxiety disorder,

● Obsessive-compulsive disorder

• Individuals who are ambulatory and non-ambulatory or who can walk with the aid of a cane, walker or leg braces for assistance with their mobility.

• Individuals who are visually impaired or hearing impaired.

• Individuals who may have needs in personal care, behavior management, money management, health care services, mobility, fine / gross motor skills, transportation, accessing community services, domestic skills, and medication management.

Each resident will have adequate access to staff for the purpose of guidance, supervision, training, and assistance. Residential Support Staff will be readily available for service delivery 7 days per week 24 hours

Each individual will be assisted in obtaining and utilizing the professional services as dictated by the assessment and their Individual Support Plan.

At QUALITY LIVING HOMES LLC, each individual is ensured a safe, sanitary, and comfortable living environment. The homes include bathrooms, dining areas, living spaces, sleeping quarters, storage rooms, laundry facilities, and leisure areas.

Each Individuals are assured appetizing and nourishing meals in accordance with the Provision of Food Services Policy.

Individuals are provided health care aides and equipment in accordance with recommendations made by licensed physicians and / or therapist.

Each Individuals will receive assistance with transferring from QUALITY LIVING HOMES LLC to a less restrictive community living arrangements or another service provider that best meets their needs when applicable.

Individuals will be provided necessary supportive services to maintain their participation in day programs or employment.
